Documented neuromuscular deficit or external immobilization.

Recently bedridden ≥ 3 days OR major surgery within 12 weeks (+1.0 pt)

Bedrest, casting, or a surgical procedure requiring general/regional anaesthesia

Localized tenderness along the distribution of the deep venous system (+1.0 pt)

Pain on palpation along the deep venous axis of the symptomatic leg

Tenderness should track the deep venous anatomy, not the joint.

Entire leg swollen (+1.0 pt)

Diffuse swelling of the symptomatic leg compared to the contralateral side

Whole-leg oedema out of proportion to calf tenderness suggests proximal clot.

Calf swelling ≥ 3 cm compared to asymptomatic leg (+1.0 pt)

Measured 10 cm below the tibial tuberosity

Use a tape measure — a hand-feel estimate is not adequate.

Pitting edema (or stronger) confined to the symptomatic leg (+1.0 pt)

Confined to the symptomatic leg, not bilateral

Bilateral pitting edema argues against an isolated DVT.

Collateral superficial (non-varicose) veins (+1.0 pt)

Prominent non-varicose superficial veins visible on the symptomatic side

A sign of deep venous collateral flow around an obstruction.

Alternative diagnosis at least as likely as DVT (-2.0 pt)

A non-DVT explanation that explains the leg findings at least as well
